Barnsley Hospital, in collaboration with the local PCT, has implemented a telecare system to assist 42 patients with Chronic Heart Failure in their own homes. As part of a research study the project includes 20 patients in
a control group and 33 carers. The presentation described:

  • how a previous study identified the reasons why older people change their health and care requirements — which resulted in the CHF trial;
  • the importance of designing robust evaluations;
  • the lessons learnt in implementing the CHF service; and
  • interim results with feedback from users and professionals.

Jeremy gave an overview of how low-power sensor interfaces and new wireless middleware is being developed to create a fully integrated and affordable personal health monitoring system for chronic disease management. His presentation covered:

  • how this collective innovation will result in the pilot implementation of a community care programme for heart failure in the north west region of London;
  • explore the use of mobile technologies for example real time monitoring of diabetes; and
