Barnsley Hospital, in collaboration with the
local PCT, has implemented a telecare system to
assist 42 patients with Chronic Heart Failure in
their own homes. As part of a research study the
project includes 20 patients in
a control group and 33 carers. The presentation described:
- how a previous study identified the reasons
why older people change their health and care
requirements which resulted in the CHF trial;
- the importance of designing robust
evaluations;
- the lessons learnt in implementing the CHF
service; and
- interim results with feedback from users and
professionals.

sensor interfaces and new wireless middleware is
being developed to create a fully integrated and
affordable personal health monitoring system for
chronic disease management. His presentation covered:
- how this collective innovation will result
in the pilot implementation of a community care
programme for heart failure in the north west
region of London;
- explore the use of mobile technologies for
example real time monitoring of diabetes; and
- examine some of the barriers and how
reconfiguration of primary care services can
